What Is Achilles Tendinitis?

Achilles tendinitis is an overuse injury of the Achilles tendon – the thick band connecting your calf muscle to your heel bone. In simple terms, it means the tendon is irritated and inflamed from repeated stress.

For example, runners or athletes who repeat a toe-off motion can overload the tendon. Over time, this causes the tendon fibers to swell and hurt. It is not a strain from one sudden injury, but rather a gradual inflammatory condition from overworking the tendon.

What Are the Common Symptoms of Achilles Tendinitis?


  • Heel/Achilles pain: You may feel a sharp or dull ache in the back of your heel or up the tendon, especially during or after activity.

  • Morning stiffness: The tendon often feels stiff or sore on the first few steps after getting out of bed. Pain usually improves a bit as you move more during the day.

  • Swelling or thickening: The Achilles tendon can look or feel swollen and tender. You might even notice it feels thicker than normal.

  • Pain during exercise: Running, jumping, or other sports often trigger or worsen the tendon pain. Discomfort may increase with activity and linger the next day.

These symptoms usually affect the back of the lower leg, just above the heel. If you hear a sudden “pop” or have very sharp pain, that suggests a tendon tear.

What Causes Achilles Tendinitis? (Risk factors)?

Common risk factors include:
  • Sports and running: High-impact activities (running, basketball, tennis, dance, etc.) place heavy stress on the Achilles. Overuse in sports is a frequent trigger.

  • Tight calf muscles: If your calf muscles are tight or inflexible, they pull extra on the Achilles with each step, raising injury risk.

  • Sudden activity changes: Quickly increasing your exercise (such as a sudden jump in mileage or intensity) can overload the tendon. “Weekend warriors” who sporadically exercise often suffer Achilles problems.

  • Poor biomechanics: Flat feet or high arches can alter foot motion and put more stress on the Achilles. Excessive pronation (rolling in of the ankles) is also a factor.

  • Unsupportive footwear: Shoes with little heel support or cushioning force your tendons to absorb more shock. Worn-out shoes or training barefoot on hard surfaces can contribute to tendinitis.

Yes. Most cases improve with conservative care. Rest, therapy exercises, proper footwear, and other treatments often resolve symptoms over time. Surgery is reserved for cases that do not respond after many months. 

It varies. A mild case may get much better within a few weeks if you rest and treat it. Moderate tendonitis often takes 6–12 weeks to significantly improve. More severe or chronic cases can last several months, typically 3–6 months of rehabilitation

Ignoring it can make things worse. Without proper treatment and rest, the tendon can degenerate (called tendinosis), meaning the tendon fibers break down. Worse, the damaged tendon is at a higher risk of a full tear or rupture.

She completed her Ph.D in biomedical sciences in 2002 and is only one of few podiatrists in the United States to carry a degree of Doctor of podiatric medicine as well as Ph.D.
